The skies fill with swarms of burning insects, and the air grows thick with decay, turning familiar zones into brutal tests of endurance. There's even a new creature, the Portent of Pain, that signals a deeper level of corruption whenever it appears. These changes make Helltide battles more unpredictable but also far more rewarding, offering a constant pull between risk and payoff for players willing to face the chaos.

Belial, the Lord of Lies, lurks within The Pit, reshaping it into a dungeon full of deception and illusion. His presence is marked by the sudden appearance of Belial's Eyes—floating orbs that explode when destroyed, stunning enemies and summoning ghostly copies of the demon. Clearing enough of these illusions can lure Belial himself into battle, turning the dungeon's climb into a high-stakes encounter. Beating him grants powerful loot and glyph upgrades, but the real challenge lies in navigating the confusion—identifying what's real, what's fake, and staying coordinated as chaos unfolds.

Andariel, the Witch Queen, has corrupted the Kurast Undercity, warping its Spirit Beacons into traps that raise the danger but also boost potential rewards. Her invasion introduces special tributes that let players tweak the odds in their favor—guaranteed boss fights, richer loot, or added dungeon affixes for a tougher, more lucrative run. One of the biggest threats is the Shade of Andariel, a shadow that saps time from the clock with each hit, pushing players to balance aggression with survival. Those who overcome her corruption can earn high-tier loot and materials, making her area one of the most rewarding yet punishing endgame zones.

Azmodan rounds out the invasion as a new world boss who roams Sanctuary and can also be summoned through three special altars. Each altar ties Azmodan's power to one of the other demon lords, creating fights that mix different effects—poison from Duriel, illusions from Belial, curses from Andariel. Players must defend the summoning altars, survive waves of empowered enemies, and then face a version of Azmodan that draws strength from multiple sources. Victory brings a chest of rare loot and Corrupted Essence, which can be used to unlock further rewards, reinforcing the season's cycle of danger and opportunity.

All of these events feed into a new progression system. Enemies corrupted by the demon lords drop Purified Gifts and Corrupted Gifts, which give players Divine Favor. This currency powers up the Divine Gifts Board—a kind of seasonal reward path that unlocks bonuses and items as players keep fighting through tougher content. It encourages steady progression, experimentation with builds, and teamwork in both open-world and dungeon settings.

Each of the four demon lords changes the rhythm of the game in a unique way—Duriel with his suffocating Helltide swarms, Belial with his illusions and mind games, Andariel with her high-risk dungeons, and Azmodan with his unpredictable global assaults.
